Where do we begin…

Thursday, May 1, 2008

Well, at the prompting of Ron, I am trying to start a blog to help keep family and friends informed of what is going on with Molly. Many of you may not know this yet, but Molly was diagnosed last Thursday, (2 days after her 13th birthday) with Scheuermann’s Disease. We were a smidge floored although I suspected it but not at the level of which she has.

Let me back up a bit….Around Christmas time, Ron, Connor and I all noticed at the same time, something different about Molly’s back. She had bent over to pick something up and we saw a bend in her back where it shouldn’t have been. We all made some startled gasp which of course, freaked her out.

At first, we must have been in denial, we thought this was some muscular issue and would work itself out. In February, when it hadn’t changed, we made an appointment with the pediatrician. Our pediatrician then ordered xrays for Molly. The results came back a few days later from the radiologist that Molly had a 14 degree scoliosis curve in her lumbar area and an advanced exaggerated kyphotic curve in her thoracic area. Their suggestion was to see a spinal surgeon asap.

Well, the earliest we could get in was about 6 weeks later, which was last Thursday, May 1 at 2:15pm. Sadly, my stepmother from my youth, Vivianne, died Friday, April 25 and her scheduled very small service was slated for May 1 at 3pm in Grosse Pointe, MI. I struggled with missing the service and tried to reschedule the dr. appt but we couldn’t get another for over a month and we weren’t comfortable with that.

While at the spinal surgeon (Dr. Dale Rowe of MSU KCMS), we were informed of Molly’s diagnosis. We were told that Molly has about an 85 degree kyphotic curve. They informed us that Molly has what is called Scheuermann’s Disease.

This is what we know about Scheuermann’s Disease/Kyphosis(pronounced shoi-er-mans):

Scheuermann's Kyphosis

  • It is a genetic condition carried by a recessive gene from either Ron or I. Our boys have a 50% chance of having it and will need to be followed carefully.

  • Scheuermann’s is characterized by abnormal, wedged-shaped vertebrae that cause the thoracic spine to curve too much. Scheuermann’s kyphosis develops over time during periods of bone growth (such as puberty). It occurs when the front of the spine doesn’t grow as fast as the back of the spine and causes healthy, rectangular-shaped vertebrae to become triangular-shaped and wedged together. This causes the thoracic spine to curve more than normal. Patients become stooped forward with a bent-over posture. It shows up right after the child has a growth spurt and the spine doesn’t grow up but rather bends out.

  • Scheuermann’s is differentiated from postural kyphosis (one caused by poor posture) by the following: if patient has a curve of over 50 degrees, if a patient has three or more adjacent vertebrae that are wedged together by at least 5 degrees per segment (15 degrees total across three segments) ,and if the patient has small disc herniations through the endplates of the vertebrae (called Schmorl’s nodes).

  • It is more common in boys than in girls.

  • The condition affects children aged 13-16 yrs and the diagnosis is rarely made prior to 10 years.

  • Patients with curves greater than 70-75 degrees require surgery.

  • Surgery is a spinal fusion of all vertebra affected.

Here is another link regarding the information above…

So, now our heads are reeling and the doc proceeded to tell us that it was suggested that Molly needs an 11 level (or 11 vertebrae) spinal fusion. This would be a 6 hour surgery which would require about a weeks stay in the hospital and about 1 month of intensive recovery followed by 5 more months of recovery to get Molly back active.

We were a bit blown away and Molly was quite the trooper through it all. She started to cry after the doctors left, but not much. She had an inkling that something would have to be done back when we found out she had this. She knew her options were possibly a brace or surgery. She was very afraid of having to wear the brace and what her classmates would say. Kids can be cruel.

Now, the brace wasn’t an option and it was all overwhelming. He explained that the spinal fusion would consist of screwing in stainless screws into the pedicles of her vertebra and these would then house clips to hold the two 1.5 foot titanium rods that would be placed along her spine to straighten it. They would shave off parts of her vertebra and then add bone from either her hip or a bone bank to fuse the vertebrae all together.

We went home with our heads spinning and began to learn about this as much as possible. Ron and I agreed we definitely needed a 2nd opinion.

Since this surgery is so long and on such a tricky part of her body, we wanted to find a doc who does this surgery ALL the time.

One of the doctors we were looking into was Dr. Lloyd Hey. Dr. Hey practices out of Duke Raleigh Medical Hospital in Raleigh, North Carolina. He has a scoliosis/kyphosis clinic down there and that is all he does primarily. I had found his blog several weeks ago and was impressed with how his blog informed of his scoliosis and kyphosis patient’s surgeries with photos and details. Ron, Molly, and I went back to this site and read for quite a while. Ron became so impressed that he felt we should check this guy out. Ron then said we should see his credentials and we went to that page. Whoa! We were floored. After Ron read it, he had no doubt that this is they guy we wanted to see Molly.

Dr. Hey, in a nutshell, graduated from MIT undergrad, Harvard Medical School, did his residency at Harvard’s Orthopaedic program, a pediatric orthopedic fellowship at Boston Children’s Hospital and this was the kicker for us, a neurosurgical fellowship at Duke.

To give a little background on Dr. Hey, he was in a very severe automobile/moped accident when he was 16 years old. He almost lost his life and leg. He was in the hospital for 3 months and endured 11 surgeries. His leg had somehow lost 2 inches of bone and he had to wear a metal device called a fixator. (Sort of seems like a halo but for the leg). It was so painful for him that he drafted his own device by transferring his xrays and the fixator’s measurements to graph paper. He then created his own “fixator” after changing it from its original parallelgram shape to a more triangular shape. His father took the plans to the Grumman Machine shop where they made the new device out of metal. They then asked Dr. Hey’s doctor to put the device on him. To us, that is just amazing.

Ron and I had decided to contact this doctor. Ron filled out a consult request and sent it to them over the weekend.

Mondy, May 5, 2008

We spoke with Meredith today over the phone. She is the scheduler down at Dr. Hey’s clinic. What a very nice, sweet, young lady she was. She was so accomodating and helpful. Meredith explained to us that Dr. Hey does his surgeries,2 a day, Monday-Thursday and sees new patients on Fridays. She said that Dr. Hey is a dad of two teenage children and doesn’t want to miss one of their games and they play on Fridays, so she wanted to find us a spot on a Friday when they didn’t play. (We totally understand with 3 being in Little League) If this date didn’t work for us, he could see us on a Monday through Thursday day between surgeries.

Meredith gave us the date of Friday, May 23 and it worked great for us. Ron went and found plane tickets via cheaptickets.com and we bought them to fly in on the Thursday before. I had arranged for Connor and Matthew to stay with our good friends, the Carusos. It worked perfectly as they go to the same school and Connor plays on the same baseball team as two of their sons. The only hitch was Connor has an all day field trip to Greenfield Village that day, which I was to chaperone but that was okay.

Later that afternoon

Around 3 pm on Monday, Meredith called me back from NC and wanted to let me know that Dr. Hey wanted to offer to do several phone consults on the phone vs. having us fly down to meet him. He said that many of his out of state and international patients will send their xrays and pics to him to look at and they will just speak on the phone several times. Then if patients decide to go with him, they schedule the surgery and come in a day ahead of time for the in person visit. Then she said he was there, if we wanted to speak with him. So, Ron and I conference-called him and spoke with him for about 20 minutes. He asked questions like “How tall we were” because he wanted to gauge how much taller Molly could be. Molly is 5’3″ right now but apparently after surgery will be 1- almost 2″ taller! And he needed to figure out where she is in her growth spurts. He also asked about our boys and that we need to follow them closely. He told us he generally does adolescent surgeries done in about 3.5 to 4 hours and that her hospital stay will be about 3-4 days long depending on what she has done. This sounds better to us than a 6 hour surgery.

Molly had many concerns, of course, about this surgery and one of them involved her sports. Molly plays softball (3rd base), skis and her favorite sport is volleyball. Well, some of these sports, we were told, were going to have to give for a year. We knew it was better to get this surgery done in the Summer for several reasons. One, she could heal and be fairly ready for school. Two, it is better to get this done before she has another growth spurt and her spine bows out some more. But Molly was devastated she wouldn’t be able to play Volleyball on her 8th grade team in the fall. But when Ron asked Dr. Hey about sports, he had said he can put Molly on a fast track rehab that he uses for his high performance athletes and she could be playing Volleyball in the Fall!!!

We were extremely impressed with him after our conversation and discussed the possibility of doing these phone consults vs. going down and meeting him in person first. We are very comfortable with this and Ron went and pushed the button to cancel our plane tickets! Saved us over $1000 in travel fees!

Dr. Hey has asked me to photograph Molly in her bathing suit from the side, front, back all standing and then one of her bending forward but taken from the side. Also, he asked us to have her xrays sent to him so he could analyze them himself and tell us his diagnosis and surgery suggestions. He expected we would talk to him near the end of this week or beginning of next, depending on when the xrays arrive. I contacted Bronson yesterday to get that ball rolling and need to go down today to release them.

Here are some images to show you kids with this exact same condition as Molly. None of these are Molly though.

And this is a boy before and after correction.

Meanwhile, we are planning on scheduling Molly for a July surgery. Our plan is for her to have about half her summer to enjoy and then the other half to recuperate. We have discussed with Molly about our different options to do with the boys at that time. Initially, Ron had thought he would go down with us all and then return back to Kzoo to work and take care of the boys and leaving me with Molly in NC. But, after discussing it, we are tentatively planning on just staying down there the whole time. Molly wants both her mom and dad there and as she indicated, she really wants the boys there too. She put it in her thinking and felt that the boys would want to be there too since if it was her, she knows she couldn’t stand being at home if it was her brother going through the same thing. She would want to see him and support him. The boys would rather go with us too, than stay.

What we think we will do, although plans can change, is to be with Molly at the hospital on the first day. Ron will then be able to do outings, sight seeing, etc. with the boys on the other days and visit Molly whenever. I would stay with her and it sounds like they even have a bed for us to stay in her room with her. Ron and I could trade off too. Ron has a cousin, Kathy, and her husband Ralph, who live in Wilmington (about 2 hours away) and I know he is hoping to connect and visit with her too.

It is a lot to process. But we wanted to start a blog to keep family and friends updated. Ron had expressed that there are a lot of people out there that care about Molly and are worried for her and this is a good way to let them all know what is going on.

I will also include a few more links here for those interested in the surgery. These are videos of Dr. Hey actually performing the surgery. These two videos will show you the surgery she will have.

**Warning, they are graphic so if you aren’t into that kind of thing, don’t go. This one here shows a scoliosis surgery (almost identical in technique) with spine correction, rod bends, and bone grafting.Surgery Video #1

And this one shows the pedicle screws and the persuader that is used to correct the spine.
Surgery Video #2

This last video is to the surgery performed at Akron Children’s Hospital.

Molly’s big task now is informing her classmates of her impending surgery. She is a shy girl as it is but she does want them to know about it. Her big fear is that there are some kids who will make fun of her or that some girls and boys will say she is doing this on purpose or faking it or that she is being a baby. Her teacher has asked if she can inform the middle school and we have said yes, but Molly wants to help prepare the info to present to them. We have been looking at journeys of girls about Molly’s age who have gone through this surgery. They have made videos to show others what they have made it through and that others can get through this too. Molly would like to make one when it is all said and done, so we will document her journey for her. Sometime next week, after we find out what Dr. Hey has to say, I will go in to her class as the teacher presents the information. Fortunately, they have a projector with internet access and can present a video or website to help explain. Molly will choose what is presented with our approval.

Here is an example of one girl’s journey.

Let me just say that Molly has been an incredible trooper throughout this week. In fact, the day after her formal diagnosis, she said to me “Mom, this is just what is supposed to happen.” We have said lots of prayers and Matthew especially, blesses her every night along with “Eight Belles”.

Throughout her journey, you may leave comments for Molly or us here. You can leave questions for us too. We will check back and update more often as the time gets closer and as we know of updates. The highlighted items throughout the text are links to different pertaining sites. There are also some links on the right at the top of the blog. I think there may be a way you can subscribe to this blog to be notified of when we update it, but I’m not sure. I’ll look into that.

Thank you all for caring!
Lysanne, Ron, Molly, Connor, and Matthew

I am so sorry to hear about Molly. I am however very happy that you did seek out a second opinion. It is so important for us to be an advocate for ourselves and seek out all of the information we can when any serious health concerns come up. Ralph and I would love to have your family visit us, and if you can’t make it down to the coast we will come up to Raleigh.

I don’t know if you have discussed this with Dr. Hey or my mom but as you may or may not know, my mom has been through two serious back surgeries. She has four titanium rods and fused discs in her lower back. The first surgery they used a five inch piece of bone from her hip she had a really tough time recuperating from the operation and still has hip pain. The second surgery they were able to use donor bone and the difference in recovery was incredible.

I know that Molly’s situation is different but you may want to ask the Dr. about using donor bone. And be sure to give my mom a call. We will keep Molly in our thoughts and prayers and I know that she is a real trouper and will pull through this with flying colors. We’ll be in touch – Love, Kathy

Wow! It sounds like you have done your homework and Molly will be under the care of a gifted surgeon.

Molly is a trooper! Let me know if there is anything I can do – ANYTHING. I need to feel like I’m helping somehow.

    Molly,
If the kids at school say that you want this and that you just want attention, I will yell at them and try to take them out, it would so be worth the detention, or in school suspension, especially for you. Don’t worry about it, because I promise I will stand up for you, even to the boys. Mrs.Pearson, wouldn’t allow the other kids to say that anyway, plus they would not say that in front of your Mom or Mrs.P. Talk to you soon!!

Thank you for the blog and the information about Molly’s condition. If there is any way I can be of support, please let me know. I am sure the kids in our school will be supportive and encouraging, as they have in similar situations. A tragedy like this can also focus us on what is important in life. You will all be in my prayers. God Bless.

Molly – I’m sure you don’t remember when we first
met. You were so tiny….maybe a couple months old.
Your Mom and I were college roommates. I can hear
the strength in you as I read what your Mom has
written. When I look back at some of the challenges
life has put before me, I remember the “hidden gifts”
that appeared. I hope you find many gifts throughout
this process. We will be sending you loving thoughts

We are so sorry to hear that. I know Molly will persevere. It will make her stronger and a better parent when she is older. Bryan was familiar with all the orthopedics stuff and said it sounded like you are getting the best care. He didn’t know of that doctor but I am asking my neighbor who is also in orthopedics, at a spinal implant company here. Bry’s company sold their spine product line. We will keep up with the blog. She doesn’t seem to have pain? What a blessing. I suppose this will kill any possibility of y’all visiting this summer. My kids will be bummedJ. We miss you all.

    Take care,

Molly, I am just finding out about your illness. I am so
glad to hear that you are letting a lot of people know
about this especially at St. A’s. I don’t know if you
remember but a few years ago I had a scary stay in
the hospital too. I counted on the prayers of the
St. A’s family. THEY REALLY WORK! God will heal you
Molly. He has to because he has so much in store
for you during your lifetime. He wants you to trust
him so he can comfort you when you are afraid.

Please know that the whole Tompkins family will be
praying for you during your surgery this summer. You
are a beautiful child of God. Stay strong!!

    Molly,
We will keep you and your whole family in our prayers every day.
It’s obvious your parents have already put some much effort into
finding you wonderful doctors. I’m sure everything will go well for you.
If there is anything we can do for you or your family you know all you
need do is ask. What we have learned through our years at St. A’s
is that when the chips are down everyone there comes through, just as
    any really great family does!

    Hey Molly

I heard about your surgery and it sounds very scary but i heard that you are being very strong about going through it. i hope that it goes very safe and that you can come back to school as soon as possible. me and my family will be praying for you to get well. if you want to know my grandma is a spine nurse and does the same surgery that you go through and you can call her if you have any questions about it and my mom used to have a job where she would be in the operating room and watch spinal surgery just like yours so she could talk to you too.

    HOPE YOU GET WELL SOON

    Molly-
This whole time i was thinking about me and
what i could do to help you instead of tinking about
you and put myself in your position. I have never
had surgery before and i have no idea what its like

doctor and telling me that i was getting surgery i
have no idea what i would do! Just remember to
be strong and you WILL get through this and if you
don’t i will get sooo depressed! You are always
so nice to everyone at school. I wish i could be
like that but people have reputations and mine
wouldn’t fit in with that. Well here is more smiles for
    you: :)! -wReN bUrToN

    Molly, Lysanne, and Ron,

Our families prayers are with you guys. You should be confident that you have done your homework, and are getting the finest care available!! Coming from someone who knows – spinal fusion isn’t the end of the world (I have two rods, and six pedicale screws). I was certainly worried about loss of motion, and the long recovery, but it went by pretty quickly, and you will be amazed how your body will adapt, and let you move regardless of the rods!! Keep the positive attitude, it helps more than you realize!!

    God Bless!!

    Drew and Laura Uryga

    Hey Molly-
Sorry to hear you have to have this surgery. Don’t
worry what anybody else thinks – you’ll be fine. Wish
you the best of luck and see ya around!

    Dear Molly,

I just found out about your blog! It’s pretty cool. I’m so sorry that you have to go through this. You so don’t deserve it! I’m glad you found that great doctor in NC that can help you out. And, if I remember right, you told me the recovery doesn’t take as long either! I really hope that you will be able to play volleyball with us when we’re back in school next year, cause the team wouldn’t be the same without you!

My family and I will be praying for you everyday!

    Molly,
    It’s Cassi again!!!!

Remember to stay strong through everything because in the end, it will all work out! I loved when Wren wrote the comment about her reputation in there!! That was funny!!! If anyone says anything bad about you, Sydney, Taylor, Erin, and I will take care of it!!! We all love you and hope you get better soon!!! See you in school tomorrow!!!!
    ~ Cassi
P.S. You can still play Volleyball, can’t you? Even if the doctor says you can’t, you will because I’ve seen you play and the team won’t be the same without you!!!!

    Hey Molly!!!
When do you leave for N.C.? Sorry i haven’t posted in a while our internet was down for a few weeks! Hey Taylor, We got back from Mackinaw last tuesday! I got some stuff from monkey business! Molly, I made a bag for you to bring when you leave for N.C. but i don’t know when to drop it off cause i don’t know when you are home? Well i am sort of bummed cause Hackett is going uniform when our class is Freshmen. This upcoming year, when Megan’ll be a freshman they have to wear ID tags, then when we are freshman, the next year, we have to wear uniforms. I don’t know if its official or anything but we got something in the mail about it from Hackett. They said it would make them closer to the other CSGK schools or something. Well any ways, i am so bored all summer cause i’m not doing anything this summer except going to my grandparents once a week. SO if you need anythiing just ask! Good luck!
    -Wren Marie

P.S. Here is one hail mary:
    Hail Mary,
    Full of Grace,
    The Lord is with thee.
    Blessed art thou among women,
    and blessed is the fruit
    of thy womb, Jesus.
    Holy Mary,
    Mother of God,
    pray for us sinners now,
and at the hour of death.
    Amen.

    Bye!

    Hi,

Glad to hear Molly is doing well. My son is 15 yrs and his paediatrician has referred him to have x-rays (taking him next week). He has had back pain for around 4 yrs since having an accident on his trampoline (he fell backwards though was not seriously injured at the time).
He wakes each morning and turns left and right and we hear his back clicking and popping – he feels he has to do this to alleviate the pain. He has also recently had physiotherapy but was just advised postural exercises. I am very fearful that he has Scheurmanns and the more I look at the curvature of his spine on standing up and bending, the more I feel he probably has it! He is autistic and both a brace and surgery would be a huge deal for him and cause him no end of distress.

I’m keeping my fingers crossed it’s good news after the x-ray. Will keep you posted.

      Mandy and Mark,
I’m so sorry that it took me 6 months to see this post!!! How is your son doing now? Were your fears confirmed? Was it Scheurmann’s? My daughter has healed amazingly. She is now 17 and a cheerleader for her high school. She’s also a 3rd baseman for her high school softball team!

She looks and feels great now!

      Keep me posted,
